Thank you for visiting the Southwest Michigan Pharmacists Association (SWMPA) website. Therefore, in the interest of the confidentiality and integrity of your sensitive personal information, we ask that you please refrain from providing SWMPA or any of its members with anything that may be considered Protected Health Information, including but not limited to your medical condition(s) or any prescription medication(s) you may be taking. If you have a question pertaining to your health, medical condition, or prescription medications we encourage you to contact your own doctor, pharmacist, or nurse. Neither SWMPA or any of its members will ever solicit any protected health information from you without your prior knowledge and consent.

If, however, you choose to share any or all of your PHI with us for any reason whatsoever (you may, for example, wish to ask one of our member pharmacists — voluntarily and in lieu of your usual community pharmacist — about a drug interaction or side-effect) you can be assured it will be kept in the strictest confidence, and treated like PHI just as covered entities such as your pharmacy, doctor, or hospitals must do under HIPAA regulations. But again, we strongly discourage you from volunteering any such information with us as SWMPA is in no way legally responsible for the confidentiality of such information.
