Your Privacy

Simply stated:

Any personal information you choose to supply to the Southwest Michigan Pharmacists Association, including but not limited to your name, email and physical addresses, and phone numbers is kept in the strictest confidence. We do not sell, exchange, or otherwise provide your personal information to any outside party for any reason. We will only use your personal information for the purpose of contacting you, or to establish an account on this site.

Also, please do not consider this website or any SWMPA member (outside of his or her own professional practice) a resource with which to ask personal questions about medical condition(s) or prescription medication(s), or provide us with any such information. If you have a question or concern pertaining to your health, medical condition(s), or prescription medication(s), we encourage you to contact your own doctor, pharmacist, or nurse in their professional setting. For much more detailed information, see our Full Privacy Policy below.

Full Privacy Policy

Thank you for visiting the Southwest Michigan Pharmacists Association (SWMPA) website. Because we respect the privacy of our visitors, this Privacy Policy Statement describes the information we may collect from you during your visit to our site, how we may use it, and how we will protect the information which you may opt to provide. This statement explains the efforts to balance our interests in collecting and using the information we receive from you with your need for appropriate protection and management of any personally identifiable information that you share with us. Use of this website is governed by our Terms and Conditions.

Personally Identifiable Information

Personally identifiable information includes your name, address, phone number, e-mail address or any other information which might reasonably be used to identify you individually. On occasion we may request personally identifiable information from our visitors but only when it is voluntarily provided, such as for the purpose of creating an account or signing up for a mailing list. SWMPA will not otherwise collect this information from you on our site.

At those portions of our site where we provide you with the option to input personally identifiable information, we notify you of the uses we intend to make of that information in any given instance. When SWMPA receives personally identifiable information, we may use it for reasonable purposes, just as non-Internet organizations do. For example, we may use this information to contact you, via e-mail or regular mail, to provide you with information we believe may be of interest such as a newsletter or meeting information. It might also be used for compiling data and analyses to understand and serve our users’ needs. Data is also compiled to evaluate the use and utility of the services we provide on-line. We will not sell, exchange, or otherwise provide your personal information to any outside party for any reason.

Collective/Aggregate Data

The SWMPA website may also collect information from you that is not personally identifiable. For example, we might track information about the date and time visitors access our site, the type of web browser they used, and the website from which they connected to our site. Our site may collect this information by depositing certain bits of information called “cookies” in a visitor’s computer. This technology does not collect an individual visitor’s personally identifiable information. Rather, this information is collected in an aggregate form. The cookies can tell us how and when pages in a website were visited and by how many people. We may use this information to improve our website to serve and inform you better. It also may allow you to shortcut access to points of interest on our website when you reenter our system. Leaving a “cookie” on your system has no effect on how it performs, and is a common practice among most websites today.

Health Insurance Portability and Accountability Act (HIPAA) and Protected Health Information (PHI)

Please note that The Southwest Michigan Pharmacists Association is a professional society and is not a healthcare provider. As such we are not considered a HIPAA covered entity, and are not required to follow any regulations pertaining to HIPAA. Therefore, in the interest of the confidentiality and integrity of your sensitive personal information, we ask that you please refrain from providing SWMPA or any of its members with anything that may be considered Protected Health Information, including but not limited to your medical condition(s) or any prescription medication(s) you may be taking. If you have a question pertaining to your health, medical condition, or prescription medications we encourage you to contact your own doctor, pharmacist, or nurse. Neither SWMPA or any of its members will ever solicit any protected health information from you without your prior knowledge and consent.

If, however, you choose to share any or all of your PHI with us for any reason whatsoever (you may, for example, wish to ask one of our member pharmacists — voluntarily and in lieu of your usual community pharmacist — about a drug interaction or side-effect) you can be assured it will be kept in the strictest confidence, and treated like PHI just as covered entities such as your pharmacy, doctor, or hospitals must do under HIPAA regulations. But again, we strongly discourage you from volunteering any such information with us as SWMPA is in no way legally responsible for the confidentiality of such information. As stated above, if you have a question or concern pertaining to your health, medical condition(s), or prescription medication(s), we encourage you to contact your own doctor, pharmacist, or nurse in their professional setting.

Links to Other Sites

As a resource to our visitors, SWMPA may provide links to other websites. We try to carefully choose websites which we believe are useful and meet our high standards for the accuracy and utility of information. However, because website design and content can change so quickly, we cannot guarantee the standards of every website to which we link. Likewise, we are not responsible for the content of any site other than our own. We also cannot guarantee the privacy policies of these other sites and suggest you check the privacy policies of those sites directly.

If you wish to stop receiving any e-mails or other such communications from SWMPA which may be sent to you in the future based on your request for this information, or if you have submitted personally identifiable information through this site and would like to have that information deleted from our records, or you with to cancel your account, please notify the webmaster and your wishes in these matters will be honored.

SWMPA will make every effort to maintain the accuracy and confidentiality of any personal information you supply to us. If you wish to make additions or other corrections to the information that you have sent in, please contact the webmaster.

All information transmitted to SWMPA via this web site is secure to the extent possible using existing technology. Unauthorized third parties should not be able to access it during transmission. We will store the information that you share with us securely and will take appropriate steps to protect it from unauthorized access or disclosure. While no security steps can offer 100 percent protection, we utilize state-of-the-art technology and systems to prevent unauthorized access to the information we hold. We will limit access to this information to those SWMPA personnel with a need to know. We educate our staff about their duty to protect your privacy, and provide training regarding the policy described herein.

SWMPA does not intentionally collect or retain personally identifiable information about children who are younger than 13 years of age. If your child may have submitted information to us without indicating their actual age, and you would like it removed, please contact the webmaster, and we will delete it immediately.

Any changes to this privacy policy will be communicated promptly at this location. Please check the privacy page periodically to review any changes that may have been made.
